By signing this form, you are giving IBD Relief Ltd (referred to as “we”, “us” or “the Organisation”) permission to use materials created through your participation, including video recordings, photographs, audio recordings, interview content, written stories, written contributions, transcripts, quotations, subtitles, captions and related materials, for the purposes described in this form.
Please take a moment to read the information below carefully. We want to ensure you understand how your contributions may be used and that you are comfortable with the choices available to you.
Please note that IBD Relief Ltd is a subsidiary of Motilent Limited. References in this form to IBD Relief Ltd include Motilent Limited, IBDmate, and organisations authorised by us to use the materials in accordance with the permissions you provide.
Our mission remains to support, educate and empower people affected by inflammatory bowel disease (IBD) through patient education, awareness, research, and healthcare improvement initiatives.
1. What you're agreeing to
By signing this form, you allow us to use materials created through your participation, including video recordings, photographs, audio recordings, interview content, written stories, written contributions, transcripts, quotations, subtitles, captions, written extracts and related materials (together referred to as the “Materials”), in any format and through any media now known or developed in the future.
The Materials may be used for the purposes described in this form and in accordance with the permissions you choose to grant in section 4.
You confirm that you are aged 18 or over, understand the nature and purpose of the project, and have had the opportunity to ask questions before agreeing to participate. Participation is entirely voluntary and you are free to decide not to take part.
If you are a parent or legal guardian completing and signing this form on behalf of a child, you confirm that you are the child’s parent or legal guardian and consent to their participation in the project and to the use of Materials created through their participation for the purposes described in this form.
By signing this form, you also confirm that you are aged 18 or over, have the legal authority to provide this consent, and are not aware of any obligation or restriction that would prevent you from doing so on behalf of the child.
2. Use of your name, image and personal information
We may use your name, image, likeness, voice, interview content and limited background information (for example, your age, occupation, general location, diagnosis or personal experience) in connection with the Materials and the purposes described in this form.
We will not publish personal contact information such as your home address, email address, telephone number or other private contact details without your specific permission.
By signing this form, you consent to such use. We will always aim to present your contribution accurately, respectfully and in a manner consistent with the purpose of the project.
If there is any information that you would prefer not to be included, or if you have specific preferences or restrictions regarding how your Materials are used, please let us know before publication or contact us at team@ibdmate.com. We will make reasonable efforts to accommodate such requests where possible.
3. Payment and expenses
Participation in this project is voluntary.
You may receive reimbursement of reasonable expenses (such as travel, accommodation or parking costs) and/or a participation payment, honorarium, voucher or other agreed contribution, where applicable to the project. Any such payment does not affect the permissions granted under this form.
Where a participation payment is offered, you are responsible for considering any personal tax, benefits, or financial implications that may apply to you.
Unless specifically agreed in writing, you will not be entitled to any further payment, royalties, licence fees or other compensation arising from the use of the Materials.
We are grateful for your contribution, which helps support education, awareness, research and patient support activities relating to inflammatory bowel disease and related topics.
4. How we may use the materials

5. What you should know about public use
The Materials may be made available through websites, educational platforms, presentations, publications, social media, partner organisations, or other communication channels in accordance with the permissions you have selected in this form.
While we will take reasonable care in how we use and share the Materials, we cannot control how other individuals or organisations may access, copy, download, record, reproduce, store, share, adapt, or redistribute them once they have been made available to others.
This means that copies of the Materials may continue to exist or be shared by third parties even if we later stop using them ourselves.
6. Right to withdraw your consent
If you change your mind and wish to withdraw your consent, you may contact us at any time at team@ibdmate.com.
Upon receiving your request, we will make reasonable efforts to stop using the Materials in future projects and, where reasonably practicable, to remove them from platforms and materials that we directly control.
However, we may not be able to remove or prevent further use of Materials that have already been shared with third parties, included in publications, presentations, research outputs, archived materials, downloaded by others, or otherwise distributed beyond our control.
Copies of the Materials may therefore continue to exist or be shared by others even after consent has been withdrawn.
Withdrawal of consent will not affect uses of the Materials that took place before we received your request.
7. Liability
While we will take reasonable care in organising and carrying out the project, you acknowledge that participation is voluntary and that, to the fullest extent permitted by applicable law, IBD Relief Ltd, Motilent Limited and their employees, contractors and representatives shall not be liable for any loss, damage, claim or expense arising from the use of the Materials in accordance with the permissions granted under this form.
Nothing in this form excludes or limits any liability that cannot legally be excluded or limited under applicable law.
8. Ownership and editing rights
You acknowledge that all rights, title and ownership in the Materials created or recorded in connection with the project shall belong to IBD Relief Ltd and its affiliated organisations, including Motilent Limited, IBDmate, successor organisations, and authorised partners acting on their behalf.
You grant us the right to use your name, image, likeness, voice, interview content, written contributions, biographical information, transcripts, quotations and other materials for the purposes described in this form, worldwide and in accordance with the permissions you have selected.
You acknowledge that we may edit, adapt, translate, subtitle, caption, shorten, combine, reproduce, publish, distribute, or otherwise modify the Materials where reasonably necessary for educational, promotional, research, accessibility, healthcare improvement, or related purposes.
You understand that you will not have rights of approval, editorial control, or review over the Materials before or after publication.
We will make reasonable efforts to ensure that any edits preserve the meaning and context of your contribution and are consistent with the purpose of the project.
9. Duration of consent
The permissions granted under this form are intended to continue indefinitely unless you withdraw your consent in accordance with Section 6.
This means that we may continue to use the Materials for the purposes described in this form without any fixed end date.
If you withdraw your consent, we will make reasonable efforts to stop future use of the Materials and, where reasonably practicable, remove them from platforms and materials under our direct control. However, withdrawal of consent will not affect uses that took place before your request was received, nor will it necessarily result in the removal of copies that have already been distributed, published, archived, shared with third parties, or otherwise passed beyond our control.
10. Legal stuff
This form and any dispute or claim arising out of or in connection with it shall be governed by the laws of England and Wales.
We hope that any questions or concerns can be resolved informally. However, if a dispute cannot be resolved, it shall be subject to the exclusive jurisdiction of the courts of England and Wales.
11. How we'll protect your information
We will handle your personal information in accordance with our Privacy Notice, the UK General Data Protection Regulation (UK GDPR), the Data Protection Act 2018, and other applicable data protection laws.
We will use your information only for the purposes described in this form and will take reasonable steps to keep it secure.
As part of the project, we may use trusted third-party service providers to assist with activities such as transcription, subtitling, translation, editing, storage, technical support, research, or distribution of the Materials. These providers will only process information on our behalf and under appropriate confidentiality and data protection obligations.
Where appropriate, we may also share information and Materials with trusted partner organisations in accordance with the permissions you have selected in this form.
You have rights in relation to your personal information, including the right to request access to the information we hold about you, request correction of inaccurate information, and, in certain circumstances, request deletion or restriction of processing. Some exceptions may apply where we are required to retain information for legal, regulatory, archival, educational, or research purposes.
