By signing this form, you're giving IBD Relief Ltd (referred to as "we" or "the Producer") permission to use your video, photographs, and interview for various purposes. Please take a moment to review the details below. We want to make sure you're comfortable with how your contributions will be used. Please note that IBD Relief Ltd is now a subsidiary of Motilent Limited. Our mission to support and empower people living with IBD remains unchanged.
1. What you're agreeing to
By signing this form, you allow us to use your video, photographs, audio, and interview content in any format (this includes on websites, social media, or promotional materials). These materials might be used for educational, promotional, or commercial purposes.
You hereby confirm that you are aged 18 or over, understand the project and have had the opportunity to ask any questions about it. Participation in the project is your choice and you do not have to take part if you do not want to.
If you are a parent or guardian completing and signing this form on behalf of a child, you confirm that you are the parent or guardian of the above named child and you give consent for the above named child to participate in the project and for us to use materials created from their participation for the purposes stated in this form. By signing this form you also confirm that you are aged 18 or over and that you have no pre-existing obligation which may restrict or limit your ability to sign this form, and that you are endorsing the previous paragraph on behalf of the above named child, noting they are aged under 18 and that the previous paragraph above is not directly applicable to them.

We may use your name, likeness, voice, and basic background information in connection with the materials we create, and by signing this form, you're giving us the permission to do so. However, we won't publish or share anything that you're uncomfortable with. If you do not want us to use any information for whatever reason please contact us at info@ibdrelief.com.

We want to be transparent: participating in this project is voluntary, and we do not offer financial compensation. You won't receive royalties for the use of your materials. However, we'll make sure your contributions help educate others and raise awareness and support for the cause.

We want to be clear that once your materials are made public (on websites, social media, etc.), they may be seen and shared by others worldwide. Once shared publicly, we can't control how others might use, copy, or share your materials. This means they could be reproduced, altered, or shared by others outside of our control.
6. Right to withdraw your consent
If, at any time, you change your mind and want to withdraw your consent, we understand. You can contact us at info@ibdrelief.com. However, please note that once your materials have been shared publicly (e.g. on a website or social media) or with a third party, we may not be able to take them down from any public domain or otherwise stop them from being shared or used by others. We can only stop using them for future purposes after you withdraw your consent.
7. Freedom from liability
By signing this form, you acknowledge that we have no liability to you in connection with the project, to the extent this is permitted under any applicable law.
8. Ownership
By signing this form, you acknowledge that all rights, title and ownership in any materials capturing your likeness in connection to the project (including video, photographs, and your interview) will remain the property of IBD Relief Ltd. We may edit, change, or modify the content as needed.
You agree that we may use your name, voice and biographical information for purposes in connection with the project worldwide and forever (in perpetuity).
We may edit, change, or modify the content as needed, and you acknowledge that you have no rights of approval or editorial control over any material captured in relation to the project.
We will try to keep any edits to any materials to a minimum, like editing them for clarity, length, or presentation. Please be assured that in respect of any edits we do make, we'll always keep the meaning and context respectful and true to the purpose of the project.
9. Duration of consent
Your consent is given forever (in perpetuity). This means that, unless we both agree to change it, we may continue to use the materials indefinitely. If you want to revoke your consent in the future, just let us know, and we'll make sure we stop using it for new projects, although content already in the public domain will remain as is.
10. Legal stuff
This form is governed by the laws of England and Wales. If we ever have a disagreement about this form, we'll work together to resolve it, and if needed, it will be handled in the courts of England and Wales.
11. How we'll protect your information
We'll look after your personal data in line with our privacy notice (which is available for you to view on request), the Data Protection Act 2018 and the UK's data protection rules. We'll only use your information for the purposes explained in this form and will keep it safe and secure.
